If your insurance company is listed above, it is your responsibility to verify the details of your own particular plan. You are required to present your insurance card and pay your co-pay at every visit.

If your insurance company is not listed above, we accept a fee for service at the time of your visit.  We will give you the necessary paperwork to send to your insurance company for possible out of network reimbursement.
